NEW Services at Delaware First Nation

SOAHAC is providing primary health care and mental health care services to members of the Delaware First Nation from the Delaware Nation Health Centre. Charlotte Allard, Family Doctor, sees patients every Thursday and Katrina Thibert, Nurse Practitioner, is on-site on Tuesdays, from 8:30 am to 4:30 pm. Sabrina Puente, Mental Health Counselor, is at the Health Centre providing counselling, support services and assistance with a variety of mental health needs Monday to Friday, 8:30 to 4:30.

SOAHAC was invited to the community through a partnership with the Delaware First Nation and the Delaware Nation Health Centre in order to provide better access to services for community members. These are funded by the Erie St. Clair Local Health Integration Network (Erie St. Clair LHIN) and Ontario Ministry of Health and Long Term Care.

These services are currently being offered to members of the Delaware First Nation. If you are not a member of the Delaware First Nation and are interested in accessing services at other locations, please contact one of SOAHAC’s other sites.  

Information for clients at Delaware First Nation:

When you visit the clinic, you might see the doctor or the nurse practitioner, depending on the reason. Dr. Allard and Katrina will work together to ensure you receive the appropriate high quality, culturally safe care.

Nurse practitioners are nurses with extended education who provide a full range of health services, similar to what you would receive at a family doctor’s office. Katrina can provide physical check-ups, illness screening and diagnosis, referrals to specialists, and prenatal care, can order diagnostic tests, can prescribe medications (except narcotics) and can help you manage short or long term illness or injuries.

When you come in and are registered for these services, you become a SOAHAC client. SOAHAC is committed to protecting your privacy according to privacy laws. That means that SOAHAC will house all of your files and personal health information separately from the Health Centre, and it will not be available to anyone else without your permission.
