Introducing our Integrated Care Managers
The Southwest Ontario Aboriginal Health Access Centre has recently been exploring ways to improve its service delivery at different sites. Considering SOAHAC’s recent expansion into Owen Sound and Windsor, the leadership team examined ways to improve efficiency to ultimately ensure clients are receiving the most comprehensive, high quality care possible.
As a result of deliberations with the Board and staff we have created three Integrated Care Manager positions – one for each site at Chippewa, London, and Owen Sound. These new roles support the integrity of the daily operation, function and performance of each site.The Integrated Care Managers work closely with SOAHAC’s senior leadership, engage with nearby Aboriginal communities and agencies, and support the work of staff at their site to ensure optimal information and resource sharing. They maintain high cultural safety standards; ensure that each client has a holistic, integrated care plan; and support inter-professional collaboration, such as including Traditional Healers within the circle of care. In this way, SOAHAC’s mission emanates from all our work, no matter where it is happening in the region.
